My truck showed “Hybrid Engine Malfunction contact dealer” upon start up, then “Reduced Engine Performance Check Engine” warnings. I was not warned that I should not drive at all so I took my son to school. As I was driving home up a hill around 35 mph the temperature gauge began to climb fast towards the upper limit. At the upper red limit the car warned “Engine Overheating Stop Immediately” and I pulled over. The engine began smoking as I pulled over. I have an aftermarket Air Intake for the twin turbo and an aftermarket cat-back exhaust system but neither of these parts could have caused dramatic overheating of the main engine and failure of the hybrid system. Events happened over the span of about 15 min driving from start up to breakdown.

Vehicle Information: 2025 Toyota Tundra CrewMax 1794 Edition 4x4 (i-FORCE MAX Hybrid) VIN: [XXX] Component: Power Train / Automatic Transmission / Hybrid Propulsion System Description of Defect: I have discovered an active fluid leak originating from the transmission/bell housing area on my vehicle. There are currently no dashboard warning lights illuminated, and the transmission is not yet slipping or shifting roughly. However, the vehicle is actively dripping fluid onto the ground and undercarriage components from the powertrain assembly. Safety Risk: A transmission fluid leak on a high-pressure 10-speed automatic transmission presents an immediate safety hazard. If the transmission fluid level drops too low while driving at highway speeds, it can cause sudden power loss, transmission lock-up, or catastrophic mechanical failure, increasing the risk of a crash. Furthermore, transmission fluid leaking onto hot exhaust or hybrid electrical components poses a severe vehicle fire risk. Subject: Safety Complaint – 2025 Toyota Tundra – Sudden Acceleration Delay – T-SB-0032-26 – Two Near-Accidents. I am the owner of a 2025 Toyota Tundra and am filing this complaint to report a serious safety concern involving delayed acceleration from a stop or near-stop condition. My vehicle has experienced this problem on two separate occasions. When I attempted to accelerate, the truck did not respond as expected, creating a dangerous situation and nearly causing a car accident on both occasions. The delay in acceleration left me unable to move forward as quickly as required by the traffic conditions. I am concerned that this issue could result in a serious collision, injury, or loss of life. I understand that Toyota issued Technical Service Bulletin T-SB-0032-26, titled “Hesitation from a Rolling Stop,” addressing hesitation from a rolling stop or quick stop on certain Toyota Tundra vehicles. My experience appears consistent with the condition described in this bulletin. I took my truck to Toyota Service in [XXX], on two separate occasions to have this issue inspected. On both occasions, the service department declined to identify or address a problem, stating that there was nothing wrong with the vehicle. As a result, I remain concerned that the safety issue has not been properly investigated or resolved. I believe this matter warrants NHTSA's attention because an unexpected delay in acceleration can create a dangerous situation when entering traffic, crossing intersections, or responding to changing road conditions. I respectfully request that NHTSA Review this complaint and investigate the delayed-acceleration issue affecting my 2025 Toyota Tundra. Examine whether the condition described in Toyota Technical Service Bulletin T-SB-0032-26 represents a safety-related defect. Here's your description, about 1,750 characters, so it fits the 2,000 limit: --- My 2025 Toyota Tundra has two intermittent defects that have not been repaired despite multiple visits to Berglund Toyota in [XXX] 1. Driver's door will not unlock. Intermittently, when I press unlock on the key fob, all doors unlock except the driver's door. I have video showing me standing outside the truck with the fob in hand while the driver's door stays locked and the other doors open normally. The dealer disassembled the door and checked the wiring and connections but could not duplicate the problem and found no codes. A Toyota field technician also inspected the truck. No parts have been replaced because the failure could not be reproduced in the shop. When this happens I cannot get into the driver's seat of my vehicle, which is a safety concern, especially in an emergency or bad weather. 2. Tailgate fails in two ways. Sometimes when raised it will not latch closed. Other times it becomes stuck in the down position and cannot be raised, even manually. On one occasion this happened in a store parking lot, and I had to secure my load with a cargo net across the open bed to drive home. The truck has been to the dealer for the tailgate more than once. The dealer has ordered latch parts, but it has not been confirmed that this will correct the stuck-down condition. Both problems are intermittent and can occur at any time. My wife and I are in our [XXX] and rely on this vehicle. Toyota has been notified in writing. Other 2025 Tundra owners have reported the driver's door locking when the tailgate is operated, which may indicate the two problems are related. Root cause: UNKNOWN. Two issues happening at the same time. “Key not detected” and “TPMS System Malfunction”. Key not detected: Happens at various times, either at startup or while driving, an error message that says “Key not detected “ comes on the dash and a loud beeping. My concern is that I may become stranded somewhere if the system cannot detect the key when trying to start the truck. The issue has been confirmed by both the dealer and Toyota and has not been fixed to date. TPMS System Malfunction: At various times while driving, a warning message is displayed on the dashboard that states there is a malfunction with the tire pressure monitoring system and that it should be checked by the dealership. My concern is that the TPMS system is unable to monitor the tire pressure and that could cause a dangerous situation. The issue has been confirmed by both the dealer and Toyota and has not been fixed to date. The truck has been back to the dealership 5 times for these issues and has been there for four days as of this writing.

Possible issue with the safety of others: My tailgate for my 2025 Tundra crewmax came opened (unknown to me) and contents of my Truck bed dumped onto the road. The incident as it happened. I was backed into a parking spot, truck was off, when I got in my remote was in my pocket and the truck would not start. So I pulled the remote out of my pocket and put it in the console. The truck would then start. I wasnt observant to the dashboard warnings if they were there. While driving home I heard all my bed contents fall out. Fortunatly I noticed it before it affected anyone. The best I can figure is that other objects in my pocket may have pushed the tailgate button. Since I was backed in and the tailgate lowering is so quiet, I didnt hear it or see it. I feel unsafe now to carry anything in my truck bed without backup protection. I would rather have a manual tailgate and totally disable the auto function but as I understand it, only the auto close mode can be disabled. I have not presented this to the dealership yet nor have I tried to duplicate the issue yet.

On 06/02/2026, at approximately 20,000 miles, the driver's front interior door handle on my 2025 Toyota Tundra TRD Pro failed. An internal component connecting the handle to the latch mechanism broke, and the door could not be opened from inside. I was unable to exit the vehicle through the driver's door using the interior handle. I had to lower the window and open the door using the exterior handle to exit the vehicle. The vehicle was repaired under warranty at Loyalty Toyota (Chester, VA), repair order 7245169/1. The dealer initially suggested the failure might be classified as improper use. They later informed me that Toyota considers it a known manufacturing defect, and the handle was replaced. On 09/06, at approximately 25,000 miles, the driver-side rear interior door handle failed in the same manner. The vehicle is currently at the same dealer awaiting parts, repair order 7255879/1. I asked the dealer to replace the remaining interior door handles because the failure is a known defect. The dealer said Toyota will replace each handle only after it fails. This is a safety concern. The interior door handles are the primary way for occupants to exit the vehicle. If a handle fails during a crash, fire, submersion, or other emergency, an occupant may be unable to open the door and could be trapped. Passengers, including children or anyone unfamiliar with the vehicle, are unlikely to know to use the window and exterior handle as a workaround. Because the failures occur without warning, there is no way to know which remaining handle will fail next. Other owners of 2025 Tundra's report the same failure on owner forums, and similar interior handle failures have been reported on the 2025 Tacoma. I am asking NHTSA to review whether this is a defect pattern affecting 2025 Tundra and Tacoma interior door handles that warrants investigation or a recall.

When using the cruise control and lane keeping system the truck will jerk into oncoming traffic and try to swerve off to turning lanes. I took it to Toyota and they couldn't find any problem. no warning lights or any indication of a problem shows on the dash. At this point the system is unusable. I have seen many post of similar incidents on the internet.
